Neck Pain Radiating to the Arm: Why It Happens and What to Do

Woman at an office desk holding her neck, with pain spreading into her arm

"My problem is in my neck, so why does my hand tingle?" Many patients who come to see me in Hyderabad ask this. The answer lies in how the nerves of the spine are wired. A problem in the neck can be felt far away, in the shoulder, arm or fingers. Doctors call this radiating pain, and when it comes from a nerve in the neck, cervical radiculopathy.

The same thing happens lower down. A problem in the lower back can send pain down the leg, which most people know as sciatica.

How the spine and nerves are arranged

Your spine is a column of bones called vertebrae, stacked one over the other. Between them sit soft discs that act as cushions and allow movement. Behind the bones and discs runs the spinal cord, and from it, a pair of nerves leaves the spine at every level.

  • The nerves that leave the neck travel to the shoulders, arms, hands and fingers.
  • The nerves that leave the lower back travel to the hips, legs and feet.

Each nerve carries two kinds of messages: sensation (feeling) and strength (movement of muscles).

Why a neck problem is felt in the arm

When a disc bulges, or the joints of the neck wear and form small bony spurs, they can press on or irritate a nerve where it leaves the spine. The brain then feels the problem along the whole path of that nerve. So the pain is felt in the arm, even though the cause is in the neck.

Common causes include:

  • a bulging or slipped disc in the neck,
  • wear and tear (cervical spondylosis) with bone spurs,
  • narrowing of the space where the nerve exits (foraminal stenosis),
  • less commonly, injury, infection or a growth.

Symptoms: from tingling to weakness

The nerve usually shows trouble with feeling first:

SymptomWhat you may notice
PainSharp, shooting or burning pain from the neck or shoulder blade down the arm
Tingling"Pins and needles" in the arm, hand or certain fingers
NumbnessA dull, dead feeling in part of the arm or fingers
BurningA hot, burning feeling along the arm
WeaknessDropping things, weak grip, difficulty lifting the arm

The pattern of fingers involved often tells the doctor which nerve level is affected, for example the thumb side, the middle finger or the little finger side.

If the pressure is severe or long-standing, weakness can appear. Weakness is a sign that the nerve needs attention soon.

The same story in the lower back: sciatica

If a disc in the lower back presses on a nerve, the pain travels down the buttock and back of the thigh into the leg and foot. That is sciatica. You can read more on our sciatica treatment page.

How it is diagnosed

  1. History and examination: your spine surgeon checks sensation, reflexes and the strength of different muscles.
  2. MRI scan: shows discs, nerves and the spinal cord, and is the main test when symptoms do not settle or there is weakness.
  3. X-rays: show alignment and wear.
  4. Nerve conduction tests: sometimes used to separate a neck nerve problem from a nerve trapped at the elbow or wrist, such as carpal tunnel syndrome.

Treatment: most people get better without surgery

Most cases of radiating arm pain improve within weeks to a few months with:

  • staying gently active and avoiding long hours with the head bent over a phone or laptop,
  • medicines prescribed by your doctor for pain and nerve pain,
  • physiotherapy with posture and neck exercises,
  • in selected patients, an image-guided injection near the nerve. See pain management services.

Surgery is considered when the arm pain is severe and does not settle with good treatment, when there is clear or worsening weakness, or when the spinal cord itself is being pressed. Common operations include removing the disc and fusing the level, disc replacement, or freeing the nerve from behind. Read more about cervical spine surgery and disc surgery.

Everyday habits that ease neck and arm pain

Man working upright with his laptop raised to eye level
  • Raise your screen. Keep your laptop or monitor at eye level and hold your phone higher instead of bending your neck down for long periods.
  • Take breaks. Every 30-45 minutes, roll your shoulders, gently tuck your chin and look up from the screen.
  • Sleep well supported. Use a pillow that keeps your neck in line with your body, not propped high.
  • Carry evenly. Avoid heavy bags on one shoulder.
  • Plan breaks on long drives while your arm pain is active.
  • Avoid forceful neck cracking or deep massage when you have arm symptoms; get a diagnosis first.

When to see a spine surgeon

Book an appointment if you have:

  • arm pain or tingling lasting more than a few weeks,
  • pain that wakes you at night or stops your work,
  • numbness that is spreading.

Get medical attention straight away if you have new or worsening weakness in the arm or hand, clumsiness of both hands, difficulty walking or balancing, or any change in bladder or bowel control. These can mean pressure on the spinal cord.

In Hyderabad, you can see me at CARE Hospitals, HITEC City or DIA Clinics, Kokapet, or book a consultation.

Frequently asked questions

Can a neck problem cause pain in the arm and fingers? Yes. A disc bulge or bone spur in the neck can press on a nerve that travels to the arm, so the pain and tingling are felt there.

Is tingling in the hand always from the neck? No. Nerves can also be trapped at the elbow or wrist, so an examination and sometimes a nerve test are needed.

Will I need surgery for radiating arm pain? Most people improve without surgery; it is considered for severe pain that does not settle, weakness, or spinal cord pressure.

Which doctor should I see for neck pain going down the arm? A spine surgeon such as Dr. Praveen Goparaju in Hyderabad can examine you, read your MRI and explain all the options.

Can a bad pillow cause arm pain? A poor sleeping posture can worsen neck pain, but radiating arm pain usually means a nerve is irritated and should be checked.
