In spondylolisthesis one vertebra slips forward over the one below it. It can cause low back pain and, when the slip narrows the space for the nerves, leg pain or numbness.
A vertebra can slip because of wear in the discs and joints with age (degenerative spondylolisthesis), a small stress crack in the back of the vertebra (isthmic spondylolisthesis) or, less often, injury.
Many slips are mild and are treated without surgery. When pain, nerve symptoms or instability continue, surgery can free the nerves and hold the spine steady.
Conditions it covers
Degenerative spondylolisthesis
Isthmic spondylolisthesis (pars defect)
Slip with spinal stenosis
Low-grade and high-grade slips
02
Is this for you?
You may benefit from an assessment if you have:
Low back pain, often worse on standing or bending backwards
Pain, numbness or tingling down one or both legs
Tight hamstrings or stiffness
Leg pain or heaviness on walking
03
How it is assessed
An examination, standing X-rays and flexion-extension X-rays to measure the slip and see if it moves, and an MRI to check pressure on the nerves.
Clinical and neurological examination
Standing and flexion-extension X-rays
MRI, and CT when needed
04
Treatment options
Non-surgical options come first. Surgery is advised only when it is likely to help.
Non-surgicalMedicines and activity changesPain relief and avoiding movements that strain the slipped level.
Non-surgicalPhysiotherapy and core strengtheningExercises that support the spine and ease pain.
Non-surgicalInjectionsNerve root blocks can calm leg pain.
SurgicalDecompression and fusion with screwsFrees the nerves and fixes the slipped level with screws and rods, often through a minimally invasive approach (MIS-TLIF), for suitable patients.
05
Recovery
After treatment, the team builds a recovery plan around you.
1Walking soon after surgery
2Pain relief
3Physiotherapy
4Follow-up X-rays

★★★★★
I had unbearable back pain for more than a year. I met many doctors but got no relief. Finally, Dr. Praveen diagnosed me with a spondylolisthesis problem and performed MIS TLIF surgery. The results are amazing — I can move freely now. He is a great spine surgeon with a very caring nature.

Questions
Can a slipped vertebra go back by itself?
No, the slip does not usually go back, but many people with a mild slip have little pain and do well with physiotherapy and core strengthening.
When is surgery needed for spondylolisthesis?
When back or leg pain continues despite proper non-surgical care, when there is weakness or numbness, or when the slip is unstable or getting worse.
Is spondylolisthesis the same as a slip disc?
No. In a slip disc the soft disc bulges out; in spondylolisthesis the bone (vertebra) itself slips forward. Both can press on nerves.
